Effect of Bushen Tongluo Decoction Combined with Extracorporeal Shock Wave Therapy on Patients with Kidney Deficiency and Blood Stasis Type of Avascular Necrosis of Femoral Head

Abstract：Objective To explore the effect of Bushen Tongluo decoction combined with extracorporeal shock wave therapy on TCM syndrome scores and serum levels of bone morphogenetic protein-2(BMP-2)and vascular endothelial growth factor(VEGF)in patients with avascular necrosis of the femoral head of kidney deficiency and blood stasis type.Methods A total of 112 patients with kidney deficiency and blood stasis type avascular necrosis of the femoral head admitted to the Navy Qingdao Special Service Recuperation Center and Yantaishan Hospital of Yantai from February 2022 to February 2025 were selected.They were divided into control and study groups via sealed-envelope randomization,with 56 cases in each group.The control group received extracorporeal shock wave therapy,while the study group received additional Bushen Tongluo decoction based on the control group's treatment.After 3 months of treatment,the following were compared between groups:efficacy,TCM syndrome scores,serum BMP-2 and VEGF levels,hip function(Harris score),pain status[visual analogue scale(VAS)],and adverse reactions.Results The total effective rate was 96.43%in the study group,significantly higher than 83.93%in the control group(P<0.05).After treatment,both groups showed reduced scores for hip pain,persistent pain,joint stiffness/limited flexion-extension,soreness/weakness of waist/knees,walking fatigue,and dizziness/tinnitus compared to pre-treatment(P<0.05),with lower scores in the study group(P<0.05).Serum BMP-2 and VEGF levels increased in both groups post-treatment(P<0.05),with higher levels in the study group(P<0.05).Harris scores increased(P<0.05)while VAS scores decreased(P<0.05)in both groups,with better outcomes in the study group(P<0.05).No significant difference was found in total adverse reaction rates between groups(P>0.05).Conclusion Bushen Tongluo decoction combined with extracorporeal shock wave therapy significantly improves clinical outcomes in kidney deficiency and blood stasis type avascular necrosis of the femoral head.It effectively alleviates TCM syndromes,enhances serum BMP-2 and VEGF levels to promote bone tissue and blood supply repair,facilitates hip function recovery,reduces pain,and does not increase adverse reactions-thus demonstrating significant clinical value.
