Clinical Value of RAR and hs-CRP/HDL-C in Predicting Coronary Restenosis after PCI in Patients with Coronary Heart Disease

Abstract:Objective To analyze the clinical value of the red blood cell distribution width-to-albumin ratio(RAR)and the high-sensitivity C-reactive protein-to-high-density lipoprotein cholesterol ratio(hs-CRP/HDL-C)in predicting coronary restenosis after percutaneous coronary intervention(PCI)in patients with coronary heart disease.Methods Patients with coronary heart disease treated with PCI at The Affiliated Hospital of Guizhou Medical University from May 2022 to January 2024 were consecutively enrolled.Among them,33 patients who developed coronary restenosis within one year postoperatively were designated as the restenosis group,and 145 patients without restenosis were designated as the non-restenosis group.All patients underwent preoperative testing for red blood cell distribution width(RDW),albumin(ALB),high-sensitivity C-reactive protein(hs-CRP),and high-density lipoprotein cholesterol(HDL-C),with RAR and hs-CRP/HDL-C subsequently calculated.General data,laboratory indicators,RAR,and hs-CRP/HDL-C were compared between groups.The predictive value was assessed using receiver operating characteristic(ROC)curve analysis,and influencing factors were identified via multivariate logistic regression analysis.Results The restenosis group had higher peripheral blood levels of RDW,hs-CRP,homocysteine(Hcy),cystatin C(CysC),RAR,and hs-CRP/HDL-C than the non-restenosis group(P<0.05),but lower stent diameter,HDL-C,and ALB(P<0.05).ROC curve analysis showed the AUC(95%CI)for predicting restenosis were 0.841(0.791-0.891)for RAR,0.854(0.804-0.904)for hs-CRP/HDL-C,and 0.904(0.854-0.954)for their combination.The combined AUC was significantly higher than that of either single indicator(vs RAR:Z=2.731,P=0.006;vs hs-CRP/HDL-C:Z=2.487,P=0.013).Multivariate logistic stepwise regression analysis identified Hcy≥14.88 ng/mL(OR=1.944),CysC≥1.88 mg/L(OR=2.678),RAR≥0.45(OR=3.618),and hs-CRP/HDL-C≥2.33(OR=4.721)as risk factors for restenosis(P<0.05).Conclusion Markedly elevated RAR and hs-CRP/HDL-C are closely associated with coronary restenosis after PCI in coronary heart disease.patients.Both ratios effectively predict postoperative restenosis,with their combination demonstrating the highest predictive value.
