Abstract:Objective To analyze the correlation between the Advanced Lung Cancer Inflammation Index(ALI)and the prognosis of advanced cervical cancer patients. Methods Clinical data of 120 advanced cervical cancer patients admitted to the Affiliated Hospital of Xuzhou Medical University from February 2020 to February 2022 were retrospectively analyzed.Body mass index was calculated, and serum albumin, neutrophil count, and lymphocyte count were measured after admission to calculate ALI.The optimal cut-off value of ALI was determined using ROC curve analysis.Patients were divided into a low ALI group(46 cases)and a high ALI group(74 cases)according to the optimal cut-off value. The relationship between ALI and pathological characteristics/prognosis was analyzed.Multivariate Cox proportional hazards regression was used to analyze prognostic factors. Results ROC curve analysis showed the optimal ALI cut-off value was 42.55,with an AUC of 0.910(95%CI:0.860-0.960),sensitivity of 72.66%,and specificity of 78.54%.The low ALI group had higher proportions of FIGO stage Ⅳ,low differentiation, lymph node metastasis, tumor diameter>5 cm,and invasion depth>1/2 myometrium than the high ALI group(P<0.05).Median survival time was shorter in the low ALI group than in the high ALI group(P<0.01).The 3-year overall survival rate was lower in the low ALI group(30.43%vs 51.35%,P<0.05).Multivariate Cox analysis showed low differentiation(HR=2.423,95%CI:1.602-3.664,P<0.01),lymph node metastasis(HR=1.900,95%CI:1.367-2.641,P<0.01),invasion depth>1/2 myometrium(HR=3.391,95%CI:1.865-6.164,P<0.01),and ALI≤42.55(HR=3.789,95%CI:2.016-7.122,P<0.01)were risk factors for 3-year overall survival. Conclusion Low ALI levels are closely associated with pathological characteristics and poor prognosis in advanced cervical cancer patients, suggesting its potential as an effective prognostic biomarker.
