Abstract:Objective To investigate the efficacy and safety of long-duration pulsed radiofrequency(LDPR)of the dorsal root ganglion combined with paravertebral nerve block(PVB)in middle-aged and elderly patients with postherpetic neuralgia(PHN).Methods A retrospective analysis included 106 middle-aged and elderly PHN patients treated at Jiangyou 903 Hospital from January 2022 to January 2025.Patients receiving conventional radiofrequency treatment were assigned to the control group(n=53),while patients receiving dorsal root ganglion LDPR combined with PVB were assigned to the observation group(n=53).Clinical efficacy, changes in Visual Analog Scale(VAS)and Pittsburgh Sleep Quality Index(PSQI)scores, and the incidence of adverse reactions were evaluated in both groups. Results The total effective rate was 88.68%in the observation group, significantly higher than 71.70%in the control group(P<0.05).At post-treatment time points T1~T4,VAS and PSQI scores decreased in both groups, with scores consistently lower in the observation group compared to the control group.Significant group main effects, time main effects, and interaction effects were observed(P<0.05).No serious adverse reactions occurred in either group during treatment. The difference in the total incidence of adverse reactions was not statistically significant(P>0.05).Conclusion Dorsal root ganglion LDPR combined with PVB demonstrates good clinical efficacy and safety in treating middle-aged and elderly PHN patients, indicating significant clinical application value.
