Relationship between Serum Uric Acid/Serum Creatinine Ratio and Disease Conditions and Poor Pregnancy Outcomes in Pregnant Women with Gestational Diabetes Mellitus

Abstract:Objective To investigate the relationship between serum uric acid(SUA)/serum creatinine(Scr)ratio and disease severity and poor pregnancy outcomes in pregnant women with gestational diabetes mellitus(GDM).Methods From January 2023 to March 2024,888 pregnant women who underwent examination and delivered at Dongguan People's Hospital were prospectively selected.At 20-28 weeks of gestation,they were divided into GDM group(249 cases)and normal group(639 cases)based on 75 g oral glucose tolerance test results.The GDM group was further divided into poor pregnancy outcome group(106 cases)and good pregnancy outcome group(143 cases).Factors influencing GDM occurrence and poor pregnancy outcomes were analyzed.The relationship between SUA/Scr ratio and disease severity was evaluated.ROC curve analysis was used to assess the predictive value of SUA/Scr ratio for poor pregnancy outcomes.Results Compared with the normal group,the GDM group had significantly higher age,urinary microalbumin,SUA/Scr ratio,hemoglobin,white blood cell count,platelet count,total cholesterol,and cesarean section rate(P<0.05),but lower gestational weight gain,free thyroxine,total bilirubin,and vaginal delivery rate(P<0.05).Low free thyroxine and high SUA/Scr ratio were independent risk factors for GDM(OR=0.523,95%CI:0.326-0.841,P<0.05;OR=2.248,95%CI:1.329-3.801,P<0.05).SUA/Scr ratio in GDM patients showed positive correlations with thyroid stimulating hormone,free triiodothyronine,and white blood cell count(P<0.05),and negative correlation with free thyroxine(P<0.05).The poor pregnancy outcome group had higher SUA/Scr ratio and total cholesterol,but shorter gestational weeks and days of gestation than the good pregnancy outcome group(P<0.05).High SUA/Scr ratio was an independent risk factor for poor pregnancy outcomes in GDM patients(OR=1.982,95%CI:1.226-3.203,P<0.05).The AUC of SUA/Scr ratio for predicting poor pregnancy outcomes was 0.787.At a cut-off value>3.37,the Youden index was 0.447 with sensitivity and specificity of 72.64%and 72.03%respectively.Conclusion SUA/Scr ratio correlates with disease severity and poor pregnancy outcomes in GDM patients,serving as an independent risk factor for both GDM development and adverse pregnancy outcomes.
