Correlation Between Matrix Metalloproteinases and Prognosis in Knee Osteoarthritis Patients Treated with Arthroscopic Debridement Combined with Platelet-Rich Plasma

Abstract:Objective To investigate the correlation between the dynamic changes of serum tissue inhibitor of metalloproteinase-1(TIMP-1)/matrix metalloproteinase-9(MMP-9)ratio and functional prognosis in patients with early-to-mid-stage knee osteoarthritis(KOA)after arthroscopic debridement combined with platelet-rich plasma(PRP)therapy.Methods A retrospective study included 120 patients with KOA admitted to the Second Affiliated Hospital of Xingtai Medical College from September 2023 to September 2024,all of whom underwent arthroscopic synovial and cartilage debridement combined with PRP treatment.Patients were grouped based on the trajectory of changes in TIMP-1/MMP-9 levels.The TIMP-1/MMP-9 levels were compared before treatment,after the second PRP session,and after the third PRP session in each group,and the correlation between the trajectory of TIMP-1/MMP-9 changes and the functional prognosis of KOA was analyzed.Results As treatment progressed,the TIMP-1/MMP-9 ratio in patients showed a continuous upward trend(P<0.05).Based on the trajectory of TIMP-1/MMP-9 changes,patients were divided into Group T1(low TIMP-1/MMP-9 ratio with rapid increase,69 patients)and Group T2(low TIMP-1/MMP-9 ratio with slow increase,51 patients).The proportion of patients in Group T2 with K-L grade Ⅲ was higher than that in Group T1(P<0.05).The rate of excellent and good functional prognosis in Group T2 was lower than that in Group T1(P<0.05).The trajectory of TIMP-1/MMP-9 changes was positively correlated with the functional prognosis of KOA(P<0.05).When the trajectory of TIMP-1/MMP-9 changes exhibited a low-level slow increase,the area under the curve for predicting the functional prognosis of KOA was 0.878(95%CI:0.805-0.930),with a sensitivity of 78.43%and a specificity of 97.10%(P<0.05).Conclusion The trajectory of changes in the TIMP-1/MMP-9 ratio varies with the progression of arthroscopic debridement combined with PRP treatment in patients with KOA.Clinically,the functional prognosis of KOA patients can be predicted by examining the trend of TIMP-1/MMP-9 ratio.
