Influence of Sufentanil Combined with Different Doses of Ropivacaine for PIEB on Labor Process and Neonatal Apgar Score in Parturients Undergoing Labor Analgesia

Abstract:Objective To explore the influence of combined application of sufentanil and different doses of ropivacaine for analgesia on labor process and neonatal Apgar score in parturients.Methods A total of 68 primiparas requiring analge-sic intervention during delivery at the Second Affiliated Hospital of Xi'an Jiaotong University from January 2023 to December 2024 were randomly divided into high-dose group(34 cases)and low-dose group(34 cases)using a random number ta-ble.Both groups received analgesia via programmed intermittent epidural bolus(PIEB)followed by patient-controlled epidural analgesia(PCEA).The PIEB analgesic solution for the high-dose group was 0.1%ropivacaine+0.5 μg/mL sufentanil,and for the low-dose group was 0.08%ropivacaine+0.5 μg/mL sufentanil.Maternal visual analogue scale(VAS)scores were com-pared between groups immediately after lateral positioning(T0),at cervical dilation to 2 cm(T1),4 cm(T2),6 cm(T3),8 cm(T4),and full dilation(T5).Analgesic drug consumption,number of PCEA compressions,labor stages(first,second,and third stag-es),duration of analgesia,neonatal Apgar score at 5 min after birth,and incidence of adverse drug reactions during analgesia were compared.Results VAS scores at T1-T5 were significantly lower than at T0 in both groups(P<0.05),with no signifi-cant difference between groups at any same time point(P>0.05).The low-dose group had significantly higher total sufentanil consumption and more PCEA compressions than the high-dose group[(39.87±6.49)mg vs(35.23±6.56)mg;(10.09±2.31)times vs(8.52±2.29)times,P<0.05].The incidence of adverse drug reactions did not differ significantly between groups(8.82%vs 5.88%,P>0.05).Conclusion Different doses of ropivacaine under PIEB provide effective analgesia.Maintaining ropivacaine at 0.1%reduces sufentanil consumption and PCEA compressions,thereby improving the analgesic experience for parturients.
