Predictive Value of Contrast-Enhanced Ultrasound Parameters Combined with Serum CA125 in Assessing Sensitivity to Neoadjuvant Chemotherapy in Breast Cancer

Abstract:Objective To explore the predictive value of contrast-enhanced ultrasound parameters combined with serum carbohydrate antigen 125(CA125)for predicting sensitivity to neoadjuvant chemotherapy(NAC)in breast cancer.Methods A total of 110 breast cancer patients who received NAC at Xingtai Central Hospital from January 2023 to Decem-ber 2024 were selected as the research subjects.According to the efficacy criteria of NAC,they were divided into the sensitive group(patients with complete response and partial response,n=74)and the insensitive group(patients with stable disease and progressive disease,n=36).All patients underwent contrast-enhanced ultrasound examination to obtain parameters including lesion onset time,peak time,and peak intensity,and serum CA125 levels were detected.The predictive value of contrast-en-hanced ultrasound parameters,serum CA125 alone and in combination for NAC sensitivity in breast cancer was analyzed by the receiver operating characteristic(ROC)curve analysis.The influencing factors of NAC sensitivity in breast cancer were explored by multivariate logistic stepwise regression analysis.Results Values for lesion onset time,peak time,peak intensity,and levels of CA125 in the insensitive group were higher than those in the sensitive group(P<0.05).The area under the curve(AUC)of the combined detection of lesion onset time,peak time,peak intensity,and CA125 for predicting NAC sensitivity in breast cancer was 0.906,which was higher than that of individual detection for lesion onset time,peak time,peak intensity,and CA125 alone(0.669,0.682,0.751,and 0.829,respectively)(Z=11.159,8.741,9.264,and 10.843,all P<0.05).Multivariate analysis showed that Ki-67 expression level(OR=2.373),lesion onset time(OR=2.489),peak time(OR=3.155),peak inten-sity(OR=2.790),and CA125 level(OR=3.377)were independent influencing factors for NAC sensitivity in breast cancer(all P<0.05).Conclusion Contrast-enhanced ultrasound parameters and CA125 can be used as important indicators for predicting NAC sensitivity in breast cancer,and the predictive value of their combined detection is higher.
