Efficacy of Hysteroscopic Planing System Resection Combined with Mirena in the Treatment of Endometrial Polyps and Its Effect on Sex Hormones and Recurrence

Abstract:Objective To explore the efficacy of hysteroscopic tissue removal system resection combined with le-vonorgestrel-releasing intrauterine system(LNG-IUS)in the treatment of endometrial polyps.Methods A total of 194 patients with endometrial polyps at Shanxi Children's Hospital(Shanxi Maternal and Child Health Care Hospital)from Oc-tober 2023 to October 2024 were randomly divided into a control group(n=97)and an observation group(n=97).The control group received traditional transcervical resection of polyp(TCRP)combined with LNG-IUS treatment,while the observation group underwent hysteroscopic tissue removal system resection combined with LNG-IUS treatment.Operation parameters and menstrual recovery were compared between the two groups.Endometrial thickness,pictorial blood loss assessment chart(PBAC)scores,and sex hormone levels were measured before and 3 months after surgery.Postoperative complications and recurrence rates at 6 months were statistically analyzed.Results Compared with the control group,the observation group had shorter operation time(P<0.05),less distension fluid usage,and reduced intraoperative blood loss(P<0.05).The postop-erative menstrual recovery time was significantly shorter in the observation group(25.12±2.34)d than in the control group(26.45±2.41)d(P<0.05).At 3 months postoperatively,both groups showed reduced endometrial thickness(P<0.05)and lower PBAC scores(P<0.05)compared to preoperative levels.The observation group had significantly thinner endometrium(P<0.05)and lower PBAC scores(P<0.05)than the control group.Serum estradiol(E2),progesterone(P),and luteinizing hor-mone(LH)levels decreased in both groups at 3 months postoperatively(P<0.05),but there was no significant intergroup dif-ference(P>0.05).The total complication rate was 4.12%(4/97)in the observation group(with 0%recurrence)versus 12.37%(12/97)and 4.12%(4/97)respectively in the control group.Both complication and recurrence rates were significantly lower in the observation group(P<0.05).Conclusion Hysteroscopic tissue removal system resection combined with LNG-IUS effectively reduces endometrial thickness,decreases menstrual bleeding,and prevents recurrence more efficiently in endo-metrial polyp treatment.Compared with TCRP,it demonstrates advantages including shorter operation time,faster recovery,fewer complications,and no adverse effects on sex hormone secretion.
