Abstract:Objective To explore the effects of levator palpebrae superioris shortening versus frontalis fascia suspension on patients with severe congenital ptosis in different age groups. Methods A total of 150 patients with severe congenital ptosis across different age groups admitted to the First Hospital of Qinhuangdao from April 2021 to April 2025 were selected. They were divided into group A(75 cases)and group B(75 cases)using a random number table method.Group A underwent frontalis fascia suspension, while group B received levator palpebrae superioris shortening. Both groups were further stratified by age into adult group A(30 cases),child group A(45 cases),adult group B(29 cases),and child group B(46 cases).General data, surgical outcomes, and postoperative aesthetic scores were compared. Results The total effective rate in adult group A(53.33%)was significantly lower than that in child group A(93.33%),with statistical significance(χ2=16.433,P<0.05).Scores for upper eyelid contour, palpebral fissure height symmetry, and double eyelid formation in adult group A and child group B were significantly lower than those in child group A(P<0.05).Adult group B showed significantly higher scores than adult group A in upper eyelid contour[(2.49±0.28)vs(2.11±0.27)],palpebral fissure height symmetry[(2.53±0.80)vs(2.19±0.29)],and double eyelid formation[(2.54±0.23)vs(2.21±0.43)](P<0.05).Conclusion Frontalis fascia suspension demonstrates better efficacy for pediatric patients with severe congenital ptosis, while levator palpebrae superioris shortening may be more suitable for adult patients.
