A Comparative Study on the Effects of Ticagrelor or Clopidogrel on Cardiac Function and Serum MMP-9 and AngⅡ in Acute Coronary Syndrome Patients after Interventional Therapy

Abstract：Objective To investigate the influence of ticagrelor versus clopidogrel on cardiac function and serum matrix metalloproteinase 9(MMP-9)and angiotensin Ⅱ(Ang Ⅱ)in patients with acute coronary syndrome(ACS)after interventional therapy.Methods A total of 160 patients with ACS admitted to the Department of Cardiology,Jieshou City People's Hos-pital from January 2019 to July 2024 were enrolled.According to the random number table method,the patients were divided into the ticagrelor group(nicorandil combined with ticagrelor)and the clopidogrel group(nicorandil combined with clopido-grel),with 80 cases in each group.Cardiac function[left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD)],serum inflammatory indexes[interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),C-reactive protein(CRP)],serum MMP-9 and Ang Ⅱ levels before surgery and 3 days after sur-gery,and the incidence of postoperative major adverse cardiovascular events(MACE)were compared between the two groups.Results Three days after surgery,LVEF in both groups was significantly increased(P<0.05),while LVEDD and LVESD were significantly decreased(P<0.05).LVEF in the ticagrelor group was significantly higher than that in the clopidogrel group(P<0.05),while LVEDD and LVESD were significantly lower(P<0.05).Three days after surgery,serum levels of IL-6,TNF-α,CRP,MMP-9,and Ang Ⅱ in both groups were significantly reduced(P<0.05),and the levels in the ticagrelor group were significantly lower than those in the clopidogrel group(P<0.05).The total incidence of MACE in the ticagrelor group(2.50%)was signifi-cantly lower than that in the clopidogrel group(11.25%)(P<0.05).Conclusion Ticagrelor and clopidogrel can both effectively treat ACS patients after interventional therapy.Compared with clopidogrel,ticagrelor can better improve cardiac function,alle-viate the inflammatory response,and reduce the levels of serum MMP-9 and Ang Ⅱ3 days after surgery.
