The Assessment Value of Ultrasound Layer-Specific Strain Technique for Left Ventricular Systolic Function and Synchrony in Patients with Acute Coronary Syndrome Complicated by Type 2 Diabetes Mellitus

Abstract:Objective To investigate the utility of ultrasound stratified strain imaging in assessing left ventricular sys-tolic function in individuals with type 2 diabetes and coronary artery stenosis.Methods A total of 85 patients diagnosed with acute coronary syndrome(ACS),who were admitted to our institution between September 2021 and September 2024,were en-rolled inparticipants in this study.According to whether they combined with diabetes,39 patients were combined with diabetes as the study group(n=39)and 46 patients were not complicated with diabetes as the control group(n=46).General information of the two groups of patients was collected,and left ventricular systolic function was tested by ultrasound stratification technology.Results The systolic blood pressure,history of alcohol consumption,coronary artery stenosis score,triglyceride levels,fasting blood glucose,and triglyceride-glucose index were notably elevated in the study group compared to the control group(P<0.05).No significant differences were observed in other clinical parameters(P>0.05).In the apical four-chamber view,the study group exhibited a significantly higher ratio of mitral valve orifice diastolic blood flow velocity E value to ventricular septal tissue Doppler value e'than the control group(P<0.05).No statistically significant differences were found in the other conventional echocardiographic parameters between the two groups of patients(P>0.05).Furthermore,the time to peak(TTP)in the anterior septum,anterior wall,posterior wall,and inferior wall of the basal segment was considerably longer in the study group compared to the control group(P<0.05).The global longitudinal strain(GLSendo)measured in the subendocardial myocardium of the left ventricle in the study group was significantly reduced compared to the control group.Similarly,the global longitudinal strain(GLS)for the study group was also notably lower than that observed in the control group.Conversely,the peak strain dispersion(PSD)value in the study group was significantly greater than that of the control group(P<0.05).No statistical difference was observed in GLSmid and GLSepi values between the two groups(P>0.05).A negative correlation was found between PSD and the absolute value of GLS in patients from the study group(r=-0.625,P<0.01).Conclusion Ultrasonic layered strain technology facilitates the measurement of left ventricular myocardial layered strain in ACS patients with diabetes.Meanwhile,the PSD was negatively associated with GLS,and the myocardial systolic function gradually decreased with increasing blood glucose level.
