Evaluation Value of CT Multimodal One-Stop Scanning for Cerebral Blood Perfusion in Patients with Acute Ischemic Stroke

Abstract:Objective To explore the value of CT multimodal one-stop scanning in guiding cerebral blood perfusion assessment in patients with acute ischemic stroke(AIS).Methods Seventy AIS patients treated at Lu'an Traditional Chinese Medicine Hospital between February 2022 and September 2024 were selected.All patients underwent CT multimodal one-stop scanning.Cerebral blood flow abnormalities were evaluated,and perfusion parameters[cerebral blood flow(CBF),cerebral blood volume(CBV),mean transit time(MTT),time to peak(TTP)]were compared between the affected side in the core infarct area,the affected side in the ischemic penumbra area,and corresponding regions on the healthy side.Cerebral collateral cir-culation status and vascular stenosis degree were assessed.Spearman analysis was used to examine relationships between per-fusion parameters(in the affected core infarct and ischemic penumbra areas)and collateral circulation/vascular stenosis.Based on prognostic follow-up,patients were divided into a good prognosis group(46 cases)and poor prognosis group(24 cases).Per-fusion parameters in the ischemic penumbra area were compared between groups,and their predictive value for prognosis was analyzed.Results CBF and CBV in the affected core infarct area were significantly lower than those in corresponding healthy-side regions and the affected ischemic penumbra area,while MTT and TTP were significantly higher(P<0.05).CBF and CBV in the affected ischemic penumbra area were significantly lower than those in corresponding healthy-side regions,while MTT and TTP were significantly higher(P<0.05).Spearman analysis showed CBV and CBF in the affected core infarct and ischemic penumbra areas were positively correlated with Alberta Stroke Program Early CT Score(ASPECTS)but negatively correlated with vascular stenosis degree(P<0.05).MTT and TTP in these areas were negatively correlated with ASPECTS but positively correlated with vascular stenosis degree(P<0.05).The good prognosis group had significantly higher CBV and CBF but lower MTT and TTP in the ischemic penumbra area than the poor prognosis group[(2.72±0.42)mL/100 g vs(2.12±0.57)mL/100 g,P<0.05;(36.62±6.79)mL/(100 mL·min)vs(25.40±6.71)mL/(100 mL·min),P<0.05;(4.63±0.81)s vs(5.88±0.94)s,P<0.05;(11.00±3.21)s vs(16.22±4.06)s,P<0.05].The areas under the ROC curve for predicting prognosis using ischemic penumbra CBV,CBF,MTT,and TTP were 0.787,0.860,0.856,and 0.839,respectively.Conclusion CT multimodal one-stop scanning perfusion parameters correlate with cerebral collateral circulation status and vascular stenosis degree in AIS patients.They can assist in evaluating cerebral blood flow in core infarct and ischemic penumbra areas,providing reference for prognosis assess-ment.
