The Predictive Value of Serum β2-MG and CRP/ALB for Acute Recurrence after Treatment in Patients with Acute Exacerbation of Bronchial Asthma

Abstract:Objective To analyze the predictive value of serum β2-microglobulin(β2-MG)and C-reactive protein/albumin(CRP/ALB)for acute recurrence after treatment in patients with acute exacerbation of bronchial asthma.Methods A total of 126 patients with acute exacerbation of bronchial asthma admitted to our hospital from January 2021 to January 2024 were selected as research subjects and divided into a recurrence group(n=51)and a non-recurrence group(n=75)based on whether they had acute recurrence within one year after treatment.All included patients had their serum β2-MG,C-reactive protein(CRP),and albumin(ALB)levels measured upon admission,and CRP/ALB was calculated.General data,related biochemical indicators,and serum β2-MG and CRP/ALB were analyzed and compared between the groups.The predictive value of serum β2-MG and CRP/ALB for acute recurrence after treatment in patients with acute exacerbation of bronchial asthma was evaluated using ROC analy-sis.The factors influencing acute recurrence after treatment in patients were explored by multivariate logistic regression analysis.Results The recurrence group showed significantly higher disease duration,proportion of asthma family history,proportion of moderate-to-severe cases,neutrophil count,Th17/Treg ratio,CRP,CRP/ALB,and β2-MG levels than the non-recurrence group,while exhibiting lower Asthma Control Test(ACT)scores,FEV1/FVC ratio,and ALB levels(P<0.05).ROC curve analysis revealed that the AUC(95%CI)for serum CRP/ALB,β2-MG,and their combination in predicting post-treatment acute recurrence in bron-chial asthma patients were 0.838(0.788-0.881),0.850(0.800-0.900),and 0.902(0.852-0.952),respectively.The AUC for the combined(serial)diagnosis of β2-MG and CRP/ALB was significantly higher than that of either biomarker alone(Z=6.542,8.455;P<0.05).Multivariate logistic stepwise regression analysis identified ACT score<20.66(OR=1.944),FEV1/FVC<53.62%(OR=2.396),Th17/Treg≥1.38(OR=1.929),β2-MG≥4.95 mg/L(OR=3.543),and CRP/ALB≥1.86(OR=4.674)as inde-pendent risk factors for post-treatment acute recurrence(P<0.05).Conclusion Abnormal elevation of serum β2-MG and CRP/ALB is closely related to acute recurrence after treatment in patients with acute exacerbation of bronchial asthma.Both can ef-fectively predict acute recurrence after treatment.The combination of the two has a higher predictive value.
