Analysis of the Prognosis Factors inFluencing Locally Advanced Esophageal Squamous Cell Carcinoma after Radical Synchronous Chemoradiation and Chemotherapy

Abstract:Objective To analyze the factors influencing the prognosis of patients with locally advanced esophageal squamous cell carcinoma(ESCC)after radical synchronous chemoradiotherapy.Methods A total of 173 ESCC patients who re-ceived radical synchronous chemoradiotherapy in the Oncology Department of our hospital from October 2016 to October 2019 were retrospectively selected as research subjects.The radiotherapy plan for all patients was three-dimensional conformal inten-sity-modulated radiotherapy,with concurrent platinum-based systemic chemotherapy.Clinical data were collected,and patients were followed up for 5 years.The log-rank test and multifactor Cox regression model were used to analyze influencing factors of OS and DFS in ESCC patients.The Kaplan-Meier method was used to calculate the survival curves for OS and DFS.Results As of the end of follow-up,the 3-and 5-year OS rates of the entire group were 54.91%and 35.84%,respectively,with a median OS of 40.6 months;the 3-and 5-year DFS were 43.93%and 29.48%,with a median DFS of 29.4 months.Multivariate Cox regres-sion analysis showed that tumor length,T stage,N stage,TNM stage,and prescription dose were independent factors affect-ing OS,while age,T stage,N stage,TNM stage,and prescription dose were independent factors affecting patient DFS(P<0.05).Conclusion Patients with locally advanced ESCC receiving radical synchronous chemoradiotherapy can achieve satisfactory sur-vival prognosis.Tumor length,T stage,N stage,TNM stage,and prescription dose are independent influencing factors for OS.Age,T stage,N stage,and TNM stage,prescription dose are an independent influencing factor for DFS.
