Comparison of Endovascular Lithotripsy and Coronary Spinning in the Treatment of Heavily Calcified Coronary Lesions

Abstract:Objective To explore the therapeutic efficacy of intravascular lithotripsy and coronary rotational grinding in patients with severe coronary artery calcification.Methods A total of 102 patients with severe coronary artery stenosis and calcification admitted to Qingdao Hospital of Cardiovascular Diseases and Beijing Fu Wai Hospital from May to December 2022 were divided into two groups using a randomized numer table method.The control group(51 cases)received coronary rotational grinding and the experimental group(51 cases)received intravascular lithotripsy.Intraoperative complications,minimum lumen diameter(MLD),postoperative stenosis degree,and lumen gain were compared between the two groups All patients underwent coronary angiography 12 months postoperatively to compare vascular patency rates,and major adverse cardiovascular events(MACE)were recorded.Results The postoperative stenosis degree in the experimental group was lower than that in the control group,while MLD and lumen gain were higher(P<0.05).The intraoperative complications rate was 3.92%in the experimental group,versus 15.69%in the control group(χ2=3.991,P=0.046).There was no significant difference in 12-month MACE rates or target vessel patency rates between the two groups(P>0.05).Conclusion Intravascular lithotripsy shows better safety and acute clinical effects than coronary rotational grinding in treating severe coronary calcification,but whether it can replace rota-tional grinding as a first-line treatment requires further research and clinical validation.
