Application of High-precision tDCS Combined with Mandatory Exercise Therapy in the Rehabilitation of Upper Extremity Motor Dysfunction after Ischemic Stroke

Abstract:Objective To investigate the enhancing effect of high-precision transcranial direct current stimulation(HD-tDCS)combined with modified compulsory exercise therapy(mCIMT)on upper limb motor function and activities of daily living(ADL)in ischemic stroke patients.Methods Six hundred patients with ischemic stroke in subacute and chronic stages admitted to Chaohu Hospital Affiliated to Anhui Medical University from April 2022 to December 2023 were included and ran-domly divided into the experimental group(n=300,receiving conventional rehabilitation plus HD-tDCS and mCIMT)and the control group(n=300,receiving conventional rehabilitation alone).Some patients in both groups were excluded according to the exclusion criteria,181 patients in the test group and 187 patients in the control group.Rehabilitation outcomes were assessed before and after the treatment using Fugl-Meyer Upper Extremity Evaluation Scale(FMA-UE),Action Research Arm Test(ARAT),Modified Barthel Index(MBI),and Stroke-Specific Quality of Life Scale(SS-QOL).Results Before treatment,there were no statistically significant differences in FMA-UE,ARAT,MBI,or SS-QOL scores between the two groups(P>0.05),indi-cating baseline comparability.After treatment,the experimental group showed significantly higher scores than the control group in FMA-UE,ARAT,MBI,and SS-QOL(all P<0.05).Conclusion HD-tDCS combined with mCIMT significantly improves upper limb motor function in stroke patients.The underlyingmechanism may involve enhanced neuroplasticity induced by syn-ergistic central and peripheral interventions.
