The Diagnostic Value of Serum PG Ⅰ/PG Ⅱ,PLR Combined with sHLA-G in Lymph Node Metastasis of Gastric Cancer

Abstract:Objective To investigate the diagnostic value of serum pepsinogen Ⅰ/pepsinogen Ⅱ(PG Ⅰ/PG Ⅱ),platelet absolute value/lymphocyte absolute value(PLR)combined with soluble leukocyte antigen G(sHLA-G)for lymph node metasta-sis in gastric cancer.Methods A total of 66 patients who were screened for gastric cancer at Hai'an Traditional Chinese Med-icine Hospital from October 2021 to May 2024 were selected and divided into two groups based on pathological diagnosis:the gastric cancer group(43 cases)and the benign gastric lesions group(23 cases).The gastric cancer group was further divided into the lymph node metastasis group(19 cases)and the no lymph node metastasis group(24 cases).The expression levels of serum PG Ⅰ/PG Ⅱ,PLR,and sHLA-G were observed in the three groups,and the calculate the positive rate.Based on the gold stan-dard for pathological diagnosis,we evaluate the diagnostic efficacy of the three indicators for lymph node metastasis in gastric cancer.Results The expression levels of PG Ⅰ/PG Ⅱ in the benign gastric lesions group,non lymphatic metastasis group,and lymphatic metastasis group decreased sequentially,while those of PLR and sHLA-G increased sequentially.The positive rates of the three indicators also increased sequentially,and the differences were statistically significant(P<0.05).The sensitiv-ity,specificity,and accuracy of the combined diagnosis using three indicators for lymph node metastasis in gastric cancer were 94.74%,84.11%,and 87.88%,respectively,all of which were higher than those of the single diagnostic tests of each indicator.Conclusion PG Ⅰ/PG Ⅱ,PLR,and sHLA-G are all serological indicators that can be used to diagnose lymph node metas-tasis in gastric cancer,and their combined application demonstrates higher diagnostic efficacy.The combined detection of PGⅠ/PGII(reflecting gastric mucosal function),PLR(characterizing systemic inflammation and immune status),and sHLA-G(medi-ating immune escape)to construct a multidimensional diagnostic model demonstrates strong innovation.Compared with previous single or dual indicator studies,the synergistic effect of the three indicators significantly improved diagnostic sensitivity(94.74%)and accuracy(87.88%),providing a new biomarker combination strategy for the preoperative non-invasive evaluation of gastric cancer lymph node metastasis.
