The Predictive Value of Total Cholesterol/High-Density Lipoprotein Cholesterol Ratio for Major Adverse Cardiovascular Events in Elderly Patients with Chronic Heart Failure

Abstract:Objective To explore the predictive value of the total cholesterol(TC)/high-density lipoprotein cholesterol(HDL-C)ratio for major adverse cardiovascular events(MACE)in elderly patients with chronic heart failure(CHF).Meth-ods A total of 106 elderly CHF patients from January 2022 to January 2024 were enrolled as the research subjects and followed up for one year.They were divided into the non-MACE group(n=75)and the MACE group(n=31)based on the occurrence of MACE during follow-up.Demographic and clinical data,TC,and HDL-C levels were collected,and the TC/HDL-C ratio was cal-culated.The predictive value of the TC/HDL-C ratio for MACE was analyzed using ROC curve analysis;risk factors for MACE were explored via multivariate logistic stepwise regression analysis.Results TC levels and TC/HDL-C ratio in the MACE group were significantly higher than those in the non-MACE group,while HDL-C levels were lower(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of the TC/HDL-C ratio for predicting MACE was 0.917(95%CI:0.867-0.956),which was higher than those of TC alone(AUC=0.728)and HDL-C alone(AUC=0.851)(Z=9.137 and 11.759,respectively;P<0.001).The age,proportion of NYNA classs Ⅳ heart failure,and BNP levels in the MACE group were higher than those in the non-MACE group,while LVEF was lower(P<0.05).Age,NYHA class,and TC/HDL-C ratio were independent risk factors for MACE,where-as LVEF was an independent protective factor(P<0.05).Conclusion An elevated TC/HDL-C ratio is strongly associated with MACE in elderly CHF patients and demonstrates strong predictive performance for MACE in this population.
