Analysis of Infections,Drug Resistance,and Prognostic Risk Factors of Carbapenem-resistant Enterobacteriaceae in Hospitalized Patients from 2021 to 2022

Abstract:Objective To investigate the major risk factors affecting the prognosis of patients infected with carbapenem-resistant Enterobacteriaceae(CRE),providing a basis for clinical treatment.Methods Clinical data of 129 hospitalized patients diagnosed with CRE infection in the Hefei First People's Hospital from January 2021 to December 2022 were collected.Patients were divided into the cured group(98 cases)and non-cured group(31 cases).Standard microbiological Methods were used for the isolation and identification of CRE,and antibiotic susceptibility testing was performed using an automated susceptibility testing system.Data were analyzed using SPSS 29.0 statistical software,with chi-square test and multivariate logistic regression analysis used to identify risk factors affecting patient prognosis.Results Among the 129 infected patients,a total of 137 CRE strains were isolated.The main species included Klebsiella pneumoniae,Escherichia coli,Enterobacter,and Citrobacter,with the strains primarily sourced from sputum,urine,blood,and ascites.Antibiotic susceptibility tests showed that CRE had higher sensitivity rates to polymyxin B,tigecycline,and ciprofloxacin.Univariate analysis showed that there were significant differences in the age,length of hospital stay,intensive care unit(ICU)stay,invasive procedures,underlying diseases,and antibiotic course between the cured and non-cured groups.Multivariate logistic regression analysis revealed that age(OR=2.438,95%CI:1.223-4.859,P=0.011),length of hospital stay(OR=1.623,95% CI:1.329-1.982,P<0.001),invasive procedures(OR=1.978,95% CI:1.008-3.882,P=0.047),underlying diseases(OR=1.735,95% CI:1.022-2.945,P=0.041),and antibiotic course(OR=1.556,95%CI:1.128-2.146,P=0.007)were independent risk factors affecting the prognosis of patients infected with CRE.Conclusion Age,length of hospital stay,ICU admission,invasive procedures,underlying diseases and antibiotic course are important factors affecting the prognosis of CRE-infected patients.Clinical measures should focus on these risk factors to improve the cure rate.
