Application of Ultrasound-Guided QLB Combined with Butorphanol PCIA in Postoperative Analgesia of Cesarean Section Patients

Abstract:Objective To investigate the application effect of ultrasound-guided quadratus lumborum block(QLB)combined with patient-controlled intravenous analgesia(PCIA)on postoperative analgesia of patients undergoing cesarean section.Methods A total of 113 patients with cesarean section in Tianchang Hospital of Traditional Chinese Medicine,Anhui Province,were selected between January 2023 and December 2023.According to the random number table method,they were divided into control group(58 cases)and observation group(55 cases).The control group received butorphanol PCIA analgesia,whereas the observation group was combined with ultrasound-guided QLB analgesia on the basis of the control group.The time of first pressing analgesia pump after surgery,butorphanol consumption at 48 hours,additional frequency of analgesia pump usage,Visual Analogue Scale(VAS)scores,off-bed activity time,lactation time,anal recovery exhaust time,Bruggrmann Comfort Scale(BCS)score,and adverse reactions were recorded and assessed in both groups after surgery.Results Compared with the control group,the time of first pressing the analgesia pump was significantly longer in the observation group,and the butorphanol consumption and additional analgesia pump usage were significantly lower at 48 hours after surgery(P<0.05).The VAS scores in the resting and motion states in the observation group were lower than those in the control group at different time periods(6 hours,12 hours,24 hours,and 48 hours)after surgery(P<0.05).The off-bed activity time,lactation time,and anal exhaust recovery time were shorter in the observation group than in the control group,and the BCS scores were higher(P<0.05).The incidence rates of adverse reactions in the observation group were significantly lower(P<0.05).Conclusion Ultrasound-guided QLB combined with butorphanol PCIA provides significant advantages in analgesia after cesarean section,effectively improving pain relief,reducing analgesic drug consumption,and promoting postoperative recovery.This method is safe and feasible for clinical use.
