Clinical Efficacy of Modified Ventilation Mode in General Anesthesia in Percutaneous Nephrolithotomy and its Influence on Serum Indicators

Abstract:Objective To investigate the clinical efficacy of modified ventilation mode in general anesthesia in percutaneous nephrolithotomy(PCNL),and its influence on the serum indicators.Methods A total of 144 patients with bilateral kidney stones treated with PCNL in the Department of Anesthesiology,Xining First People's Hospital from May 2019 to December 2023 were selected as research subjects.They were divided into the study group and control group,with 72 cases per group.Patients in the control group were given standard ventilation in general anesthesia,and those in the study group were given low respiratory rate and low tidal volume ventilation in general anesthesia.The clinical efficacy,hemodynamic indicators(mean arterial pressure[MAP],heart rate[HR]),inflammation indicators(procalcitonin[PCT],C-reactive protein[CRP])and renal function indicators(serum creatinine[Scr],blood urea nitrogen[BUN])were compared between groups.Results The stone clearance rate,stone fragmentation rate,operation time and hospital stay of the patients in the study group were significantly better than those in the control group(P<0.05),while the end-expiratory CO2 partial pressure(ETCO2)was significantly higher(P<0.05).Compared with the pre-anaesthesia values,MAP and HR at post-anesthesia were significantly reduced in both groups(P<0.05),but there were no significant differences between the two groups(P>0.05).Compared with the pre-operative values,PCT and CRP were significantly elevated in both groups(P<0.05),and they were significantly higher in the study group than the control group(P>0.05).There were no significant differences in the renal function indexes before and after anesthesia in both groups(P>0.05).Conclusion The modified ventilation mode in general anesthesia during PCNL has a significant efficacy.It not only maintains the stability of hemodynamics and renal function,but also reduces the inflammatory response,and ultimately accelerates the recovery,which is worthy of clinical promotion and application.
