Correlation of CDK5RAP3 Expression with Clinical-pathology and its Value in Prognosis Prediction in Gastric Cancer Patients

Abstract：Objective To explore the correlation of cyclin-dependent kinase 5 regulatory subunit-associated protein 3(CDK5RAP3)expression with clinical-pathology and its value in prognosis prediction in patients with gastric cancer(GC).Methods A retrospective analysis was performed on 114 patients with GC who visited the Department of Gastroenterology of First Hospital of Qinhuangdao from February 2017 to February 2019.Postoperative specimens of the patients were subjected to immunohistochemical analysis of CDK5RAP3.The clinic-pathological correlation and the value of prognosis prediction with expression level of CDK5RAP3 were comparatively analyzed.Results No significant differences in gender,tumor location,Borrmann type,pathological type,tumor diameter,proportion of postoperative chemotherapy,and lymphatic metastasis were found between patients with CDK5RAP3 low expression and those with CDK5RAP3 high expression(P＞0.05).While,the proportions of age ≥ 60 years and pathological tumor(pT)stage Ⅲ+Ⅳ in patients with CDK5RAP3 low expression were significantly higher than those with CDK5RAP3 high expression(P＜0.05).The 5-year overall survival rates of patients with CDK5RAP3 low expression and high expression were 72.37％(55/76)and 89.47％(34/38),respectively.The death risk was significantly higher in patients with CDK5RAP3 low expression than those with CDK5RAP3 high expression(HR=3.164,95％CI:1.285-7.791,P=0.019).The expression level of CDK5 RAP3 had a high diagnostic efficiency in predicting the prognosis of patients with GC(area under the curve[AUC]was 0.886),with an optimal cutoff point of ≥4.The predictive accuracy,sensitivity,specificity,positive predictive value(PPV),and negative predictive value(NPV)for prognosis were 83.85％,93.24％,81.28％,87.15％,and 82.03％,respectively.Conclusion The expression level of CDK5RAP3 has a certain predictive value for the prognosis of patients with GC.The prognosis is poor in older and higher pT stage of GC patients with CDK5RAP3 low expression.
