Correlation between red blood cell distribution width/platelet count ratio with renal injury in children with Henoch-Schönlein purpura

Abstract:Objective To explore the predictive effect of red blood cell distribution width(RDW)-to-platelet count ratio(RPR)on renal injury in children with Henoch-Schönlein purpura(HSP).Methods A total of 142 children with HSP admitted to Hai'an People's Hospital between June 2022 and February 2024 were enrolled.The children with HSP were divided into renal injury group(n=37)and non-renal injury group(n=105)based on the presence or absence of renal injury.The coefficient of variation of red blood cell distribution width(RDW-CV)and platelet count(PLT)in peripheral venous blood were detected by automatic blood cell analyzer,and the RPR value was calculated.The receiver operating characteristic(ROC)curve was plotted to analyze the value of RPR in evaluating renal injury in children with HSP.Multivariate Logistic regression analysis was employed to analyze the related factors of renal injury in children with HSP.Results RDW-CV,PLT and RPR levels were significantly higher in renal injury group than in non-renal injury(P<0.05).The AUC values of RDW-CV,PLT and RPR for predicting renal injury in children with HSP were 0.851,0.734 and 0.923,respectively.The duration of rash recurrence and frequency of rash recurrence were significantly higher in the renal injury group than in the non-renal injury group(P<0.05).The results of multivariate Logistic regression analysis showed that the rash recurrence duration≥2 months(OR=3.165,95%CI:1.420-7.054),rash recurrence frequency≥3times(OR=3.522,95%CI:1.543-8.038),and RPR≥0.126(OR=5.089,95%CI:1.902-13.612)were risk factors for renal injury in children with HSP(P<0.05).Conclusion The high expression of RPR is related to the occurrence of renal injury in children with HSP and is a risk factor for renal injury.Comprehensive analysis of RPR demonstrates significant clinical value in evaluating renal injury in children with HSP.
