Predictive Value of Systemic Immunoinflammatory Index in Deep Vein Thrombosis of Elderly Patients After Hip Replacement

Abstract：Objective To investigate the value of systemic immunoinflammatory index(SII)in predicting deep vein thrombosis(DVT)in elderly patients after hip replacement.Methods The clinical data of 120 elderly patients undergoing hip replacement in Liuzhou Municipal Liutie Central Hospital from March 2022 to March 2024 were collected.Preoperative neutrophil count(NEU),lymphocyte count(LYM),and platelet count(PLT)were recorded,and the SII was calculated.The occurrence of DVT after operation was recorded.Patients were divided into the thrombosis group(n=23)and non-thrombosis group(n=97).The independent risk factors for DVT after hip replacement in elderly patients were investigated by multivariate Logistic stepwise regression.Results Patients in the thrombosis group had significantly higher PLT,NEU,and SII than the thrombosis group,and the LYM was significantly lower(all P＜0.05).The area under the curve(AUC,95％CI)of PLT,NEU and LYM in predicting DVT after hip replacement in elderly patients was 0.729(0.684-0.774),0.841(0.796-0.891),0.803(0.758-0.853),respectively,and that of SII was 0.917(0.872-0.967).The proportion of BMI ≥24 kg/m2,surgical time ≥2 h,postoperative bed rest ≥3 d,no early activity after surgery,history of diabetes,and history of heart failure was significantly higher in the thrombosis groupthan the non-thrombosis group(all P＜0.05).Multivariate analysis showed that BMI≥24 kg/m2(OR=2.396,95％CI:1.305-4.400),surgical time≥2 h(OR=2.759,95％CI:1.423-5.352),postoperative bed rest≥3 d(OR=2.465,95％CI:1.358-4.472),SII≥906.86(OR=4.473,95％CI:2.145-9.328)were factors influencing the incidence of postoperative DVT in elderly patients with hip replacement(all P＜0.05).Conclusion Preoperative SII can be used as an independent predictor of postoperative DVT in elderly patients with hip replacement,which is worthy of promotion and application in clinical practice.
