Analysis of Risk Factors for Deep Venous Thrombosis in Lower Limbs after High Tibial Osteotomy Combined with Arthroscopic Surgery for Knee Osteoarthritis

Abstract:Objective To study the risk factors for deep venous thrombosis(DVT)of lower limbs after high tibial osteotomy(HTO)combined with arthroscopic surgery for knee osteoarthritis(KOA).Methods A total of 43 KOA patients who underwent HTO combined with arthroscopy and developed DVT in Tangshan No.2 Hospital from January 2021 to March 2024 were selected as the DVT group.A total of 129 KOA patients who did not develop DVT after HTO combined with arthroscopy were randomly selected in the control group at a 1∶3 ratio.The gender,age,body mass index(BMI),personal history,accompanying diseases,the American Society of Anesthesiologists(ASA)physical status classification,course of KOA,Kellgren-Lawrence(K-L)grading,surgical side,anesthesia method,surgical time,intraoperative blood loss,tourniquet use time,postoperative anticoagulation,preoperative D-dimer(D-D),and postoperative bed rest time were collected.The differences in the above data between two groups of patients were observed using chi-square test,and multivariate Logistic regression analysis was used to identify the risk factors for DVT.Results There were no significant differences in the gender,alcohol consumption,hypertension,hyperlipidemia,course of KOA,K-L grading,and surgical side between DVT and non-DVT groups(P>0.05).There were significant differences in the age,BMI,smoking,diabetes,ASA classification,anesthesia mode,operation time,intra-operative blood loss,tourniquet use time,postoperative anticoagulation,preoperative D-D,and postoperative bed rest time between DVT group and non-DVT group(P<0.05).Multivariate logistic regression analysis showed that diabetes,postoperative bed rest time≥3 days,preoperative D-D≥500ng/mL and tourniquet use time ≥75 min were risk factors for DVT after HTO combined arthroscopy in KOA patients(P<0.05),and postoperative anticoagulation was protective factors for DVT after HTO combined arthroscopy in KOA patients(P<0.05).Conclusion The risk of DVT after HTO combined with arthroscopy can be elevated by diabetes,increased preoperative D-D,prolonged use of tourniquet during operation,and prolonged bed rest after operation.Postoperative anticoagulant treatment can reduce the risk of DVT.
