Significance of Radiographic Keratotic Mucosa Width and Bone Level of Implant Margin in Diabetic Nephropathy Patients with Peri-implantitis

Abstract:Objective To explore the significance of radiographic mucosa width and marginal bone level in diabetic nephropathy with peri-implantitis.Methods Clinical data of 92 cases of diabetic nephropathy with dental implants treated in the Urumqi Stomatological Hospital,the Second Affiliated Hospital of Xinjiang Medical University from May 2022 to May 2024 were retrospectively analyzed.A total of 60 patients without periimplantitis were enrolled in the control group,and 32 with periimplantitis were included in the study group.Periodontal and imaging examination data were compared.The predictive value of radiographic keratinized mucosal width and bone level of implant margin in the occurrence of periimplantitis was analyzed.Results The width of keratinized mucosa in the study group was significantly less than the control group,and the marginal bone level was significantly lower than the control group.The exploration depth and attachment loss were significantly greater than the control group(P<0.05).Logistic regression analysis showed that the width of keratinized mucosa and the bone level of implant margin were independent factors for peri-implantitis in patients with diabetic nephropathy(P<0.05).According to the receiver operating characteristic(ROC)curve analysis,the area under the curve(AUC)of keratinized mucosa width in predicting periimplantitis in diabetic nephropathy patients was 0.895,with the sensitivity of 93.75% and specificity of 95.00%.The AUC of implant margin bone level in predicting periimplantitis in diabetic nephropathy patients was 0.911,with the sensitivity of 96.88%and specificity of 93.33%.There was a negative correlation between keratinized mucosa width and peri-implantitis(r=-0.367,P<0.05),and a significant negative correlation between implant margin bone level and peri-implantitis(r=-0.439,P<0.05).Conclusion Through imaging detection,the insufficiency of keratinized mucosa and the reduction of bone level at the implant margin can be found in time,and corresponding therapeutic measures can be taken to reduce the risk of peri-implantitis.
