The Significance of the System Inflammation Response Index in the Frailty Diagnosis in the Elderly Patients with Maintenance Hemodialysis

Abstract:Objective To explore the correlation between system inflammation response index(SIRI)as an indicator of inflammationand frailty in elderly patients with maintenance hemodialysis(MHD),thus providing a simple and effective method for early identification of frailty in MHD patients.Methods A total of 72 MHD patients aged over 65 years(including 44 cases in the Second People's Hospital,of Jiaozuo and 28 in the Qinyang People's Hospital)were enrolled.They were divided into the frailty group(38 cases)and normal group(34 cases,including non-frailty and pre-frailty)according to the Fride frailty phenotype(FFP).The baseline data and the levels of SIRI,systemic immune inflammatory response index(SIIRI),systemic immune-inflammation index(SII),neutrophil-to-lymphocyte ratio(NLR)and platelet-to-lymphocyte ratio(PLR)were compared between the two groups.The receiver operating characteristic(ROC)curve was generated and the area under curve(AUC)was calculated to evaluate the ability to predict frailty.Multivariate binary Logistic regression was used to analyze the influencing factors of frailty in elderly MHD patients.Results There were significant differences in the age,body mass index(BMI),vascular access between the two groups(P<0.05).SIRI,SIIRI,SII,NLR and PLR in the frailty group were significantly higher the normal group(P<0.05).The AUC of SIRI was 0.772(95%CI:0.661-0.882,P<0.001),which was significantly higher than that of the SIIRI,SII,NLR and PLR.The maximum Youden Index was 0.485,with the optimal cutoff value of 1.27,sensitivity of 63.2%,and specificity of 85.3%.Logistic regression analysis showed that BMI was negatively associated with frailty(OR=0.751)and SIRI was positively associated with frailty(OR=8.824)in elderly MHD patients.Conclusion SIRI can predict the occurrence of frailty in elderly patients with MHD.Low BMI and high SIRI are risk factors for frailty in elderly patients with MHD.
