Correlation between Inflammatory Markers and Tumor Markers and Lung Adenocarcinoma Complicated with Spread Throdgh Air Spale

Abstract:Objective To detect inflammatory markers and tumor markers in patients with lung adenocarcinoma,and to explore the correlation between inflammatory markers and tumor markers and STAS of lung adenocarcinoma.Methods A retrospective analysis was conducted on 170 patients with lung adenocarcinoma confirmed by surgical pathology at the Second Affiliated Hospital of Chengdu Medical College from January 2021 to October 2023.Patients were divided into STAS-positive and STAS-negative groups based on STAS evaluation criteria.The clinical data,pathological data,inflammatory indicators,and tumor marker levels were compared between the groups.ROC curves were used to compare the area under the curve(AUC)for these indicators.Multivariate Logistic regression was applied to identify independent risk factors for STAS.Results Among the 170 lung adenocarcinoma cases,42(24.70%)were STAS positive.STAS-positive patients showed significantly higher NLR(4.07±0.52 vs 2.37±0.14,t=4.363,P<0.001),PLR(166.14±9.98 vs 128.01±4.56,t=3.889,P<0.001),SII(753.20±80.77 vs 457.78±32.88,t=4.012,P<0.001),CEA(5.69±1.29 vs 1.94±0.17,t=4.681,P<0.001),and SCC-Ag(1.69±0.20 vs 1.21±0.05,t=3.096,P=0.02)compared to STAS-negative patients.The ROC results showed that the area under the curve of NLR,PLR,SII,CEA and SCC-Ag was 0.900,which was higher than that of the single detection of the above indexes(P<0.05).Multivariate Logistic regression analysis showed that NLR≥2.597(OR=3.529,95%CI:1.236-10.079),SII≥547.15(OR=4.154,95%CI:1.266-14.072)and CEA≥2.54ng/mL(OR=7.092,95%CI:2.752-18.275)were independent risk factors for STAS in patients with lung adenocarcinoma(P<0.05).Conclusion NLR≥2.597,SII≥547.15,and CEA≥2.54 ng/mL are independent risk factors for STAS in lung adenocarcinoma.By combining these markers,the patient's condition and prognosis of lung adenocarcinoma can be more comprehensively evaluated,providing a basis for individualized treatment planning.
