CK,NHR and MYO in the Diagnosis of STEMI and Prediction of In-Hospital Heart Failure

Abstract:Objective To investigate the role of creatine kinase(CK),neutrophil-to-high-density lipoprotein cholesterol ratio(NHR),and myoglobin(MYO)in the diagnosis of acute ST-segment elevation myocardial infarction(STEMI)and the prediction of in-hospital heart failure.Methods A total of 117 STEMI patients admitted to Nanxishan Hospital of Guangxi Zhuang Autonomous Region from August 2021 to August 2023 were included as the observation group,and 90 healthy individuals undergoing physical examination during the same period were selected as the control group.The levels of NHR,CK,and MYO were compared between the two groups.The diagnostic performance of NHR,CK,and MYO alone and in combination for STEMI was calculated.Patients were further categorized into heart failure and non-heart failure groups based on in-hospital heart failure occurrence,and the levels of NHR,CK,and MYO were compared between the two groups.The predictive value of NHR,CK,and MYO for in-hospital heart failure was also analyzed.Results The levels of NHR,CK,and MYO in the observation group were significantly higher than those in the control group(P<0.05).The AUC for STEMI diagnosis using NHR,CK,and MYO combined was 0.894,significantly higher than that of NHR,CK,or MYO alone(P<0.05).Among the 117 STEMI patients,75 were classified as the non-heart failure group,and 42 were classified as the heart failure group.The levels of NHR,CK,and MYO in the heart failure group were significantly higher than those in the non-heart failure group(P<0.05).The AUC for predicting in-hospital heart failure using the combined detection of NHR,CK,and MYO was 0.854,which was also significantly higher than that of individual biomarkers(P<0.05).Conclusion The expression of NHR,CK,and MYO is significantly altered in STEMI and in patients with in-hospital heart failure.Combined detection of these three biomarkers offers a reliable method for diagnosing STEMI and predicting in-hospital heart failure,providing important clinical guidance.
