The Value of DPR in Predicting Deep Vein Thrombosis After Total Hip Replacement in Elderly Patients

Abstract:Objective To investigate the value of D-dimer/platelet ratio(DPR)in predicting deep vein thrombosis(DVT)in elderly patients after total hip replacement.Methods The clinical data of 365 elderly patients with total hip replacement treated in Changsha Central Hospital from January 2019 to December 2023 were retrospectively analyzed.D-dimer levels were measured by automatic biochemical analyzer,platelet counts by automatic blood cell analyzer,and DPR calculated.Doppler ultrasound was used on postoperative day 5 to detect DVT,and the patients were divided into DVT group(56 cases)and no DVT group(309 cases).Receiver operating characteristic(ROC)curve was used to assess the predictive value of D-dimer,platelet,and DPR.Logistic regression was performed to analyze influencing factors.Results The levels of D-dimer and DPR in the DVT group were higher than those in the non-DVT group,and the platelet level was lower in the DVT group compared to the non-DVT group,with a statistically significant difference(P<0.05).ROC curve analysis showed that the area under the curve(AUC)for D-dimer in predicting DVT after total hip replacement in elderly patients was 0.866(95%CI:0.815-0.918),and the AUC for platelet was 0.750(95%CI:0.699-0.802).Both were lower than the AUC for DPR,which was 0.902(95%CI:0.851-0.953).Additionally,the ages of the DVT group were higher than those of the non-DVT group.The CRP level,the proportion of patients with BMI≥24.0 kg/m2,the proportion of diabetes mellitus,the proportion of operations lasting≥90 minutes,and the proportion of prosthetic fixation Methods using bone cement were all higher in the DVT group compared to the non-DVT group.Conversely,the HDL-C level was lower in the DVT group,with statistically significant differences(P<0.05).BMI≥24.0 kg/m2(OR=2.264,95%CI:1.548-3.311),diabetes mellitus(OR=2.782,95%CI:1.825-4.239),and DPR≥1.24×10-2(OR=3.740,95%CI:2.207-6.336)were identified as independent risk factors for DVT in elderly patients after total hip replacement(P<0.05).Conclusion DPR level is an independent risk factor for DVT in elderly patients with total hip replacement and may serve as a biomarker for DVT prediction.
