Abstract:Objective To explore the factors affecting the prognosis of patients with hepatocellular carcinoma(Hepato-cellular Carcinoma,HCC)after radiofrequency ablation, and to compare the efficacy of postoperative tumor markers Alpha-fe-toprotein(Alpha-fetoprotein,AFP),Protein Induced by Vitamin K Absence or Antagonist-II(Protein Induced by Vitamin K Absence or Antagonist-II,PIVKA-II)and ultrasonography in assessment of HCC efficacy. Methods 114 patients with HCC who underwent radiofrequency ablation at The 958th Hospital of Chinese People's Liberation Army from May 2020 to May 2023 were selected and divided into a good prognosis group and a poor prognosis group, with 57 cases in each group. The patients were followed up at 6 months postoperatively with ultrasonography and tumor marker tests. The efficacy of tumor markers and ultrasonography in the diagnosis of HCC was assessed by statistical analysis comparing the postoperative tumor ablation rate and the changes of AFP and PIVKA-II in the two groups, and by using the ROC curve. Results The tumor ablation rate in the good prognosis group was significantly higher than that in the poor prognosis group(85.9%vs 56.1%,P<0.001).At 6 months postoperatively,AFP and PIVKA-II levels decreased significantly in the good prognosis group(P<0.001),whereas there was no significant decrease in the poor prognosis group. The sensitivity and specificity of combined ultrasonography and tumor mark-er detection for the diagnosis of HCC were significantly higher than that of single detection methods(88.76%sensitivity and 93.84%specificity for combined detection). Conclusion The combined tumor marker and ultrasonography assessment method after radiofrequency ablation in patients with HCC has high sensitivity and specificity in diagnosis and efficacy determination.
