Predictive Factors for Recurrence of Interstitial Lung Disease Associated with Polymyositis/Dermatomyositis

Abstract:Objective To assess the clinical features and risk factors of interstitial lung disease(ILD)in polymyositis(PM)/dermatomyositis(DM).Methods A total of 100 PM/DM patients admitted to our hospital from January 2021 to December 2023 were included.Among them,50 patients were diagnosed with ILD,and 50 patients were diagnosed without ILD.The gen-eral data,clinical characteristics,and laboratory test results of the ILD group and non-ILD group were compared and analyzed.Results Patients in the ILD group were significantly older than those in the non-ILD group(P<0.001).There was no significant difference in gender between the two groups(P>0.05).The incidence rates of arthritis,Raynaud's phenomenon,Gottron rash,fe-ver,and dry cough were significantly higher in the ILD group compared to the non-ILD group(P<0.05).There were no significant differences in disease duration,myalgia,swallowing difficulties,or exertional dyspnea between the two groups(P>0.05).Labo-ratory results showed that the positive rates of autoimmune disease-related antibodies,including anti-Jo-1 antibody,antinuclear antibody,anti-Ro52 antibody,and anti-MDA5 antibody,were significantly higher in the ILD group compared to the non-ILD group(P<0.05).Serum albumin and creatine kinase levels were significantly lower,while globulin and erythrocyte sedimentation rate were significantly higher in the ILD group(P<0.05).There were no significant differences in levels of ALT,AST,white blood cell count,lactate dehydrogenase,or C-reactive protein between the two groups(P>0.05).Multivariate logistic regression anal-ysis showed that the risk factors for ILD in PM/DM patients included age,fever,dry cough,arthritis,Raynaud's phenomenon,and positive rates of anti-Jo-1 antibody and antinuclear antibody(P<0.05).Conclusion Advanced age,fever,dry cough,arthri-tis,Raynaud's phenomenon,and positive anti-Jo-1 anti-body and antinuclear antibody are significant risk factors for ILD in PM/DM patients.During disease progression,these risk factors should be closely monitored,and effec-tive interventions should be implemented to reduce the occurrence of ILD and improve disease prognosis.
