Value of Fetal Heart Z-score and Cardiac Axis Combined with nuchal translucency and tricuspid regurgitation peak velocity in the Prenatal Diagnosis of Fetal Congenital Heart Disease in the First Trimester of Pregnancy

Abstract:Objective To explore the diagnostic effect of nuchal translucency(NT),tricuspid regurgitation(TR),mandrel value combined with fetal heart measurement parameter Z-score on fetal congenital heart disease.Methods In this study,67 cases of congenital heart disease retrospective selection of cases in our hospital in early pregnancy from January 2019 to February 2024 were used as the observation group,and the control group consisted of 67 examples of normal hearts in normal gestational weeks.All patients were diagnosed by ultrasound.The levels of Z-score,cardiac axis,NT and tr of the two groups were compared,and the cardiac Z-score,cardiac axis combined with NT The value of TR in prenatal diagnosis of fetal congenital heart disease.Results Nt of patients in the observation group(x2=93.632,P=0.000)、TR(x2=84.156,P=0.000)was significantly higher than that in the control group,and the spindle(t=5.293,P=0.000)was significantly higher than that in the control group;In the prediction of biparietal diameter,the Z-score of aorta in the observation group was lower than that in the control group(t=40.267,P=0.000),and the Z-score of pulmonary artery was higher than that in the control group(t=22.702,P=0.000).In the prediction of femoral length,the Z-score of aorta(t=24.556,P=0.000)and pulmonary artery(t=11.230,P=0.000)in the observation group were lower than that in the control group.In the prediction of actual gestational weeks,The aorta(t=32.945,P=0.000)and pulmonary artery(t=11.351,P=0.000).Z-score、The combined detection of cardiac axis,NT,and TR has higher diagnostic specificity for congenital heart disease fetuses than individual detection.Through the analysis of the working characteristic curve of the subjects,Z-score、The combined detection of cardiac axis,NT,and TR has a significantly higher area under the curve for congenital heart disease compared to individual detection..Conclusion Z-score,cardiac axis combined with NT and TR in early pregnancy have significant significance in prenatal diagnosis of fetal congenital heart disease,which is recommended to be popularized in clinic.
