Abstract:Objective To explore the relationship between the red blood cell distribution width(RDW)/albumin(ALB)ratio(RAR)and the degree of coronary artery stenosis in perimenopausal female patients with coronary heart disease. Methods A total of 128 perimenopausal female patients with coronary heart disease treated at Xingtai People's Hospital from January 2022 to January 2024 were included.Based on the degree of coronary artery stenosis(Gensini score),patients were divided into the mild-to-moderate group(n=98)and the severe group(n=30).The RAR levels were compared between the two groups, and the correlation of RDW,ALB,and RAR with the Gensini score was analyzed.Receiver operating characteristic(ROC)curve analysis and area under the curve(AUC)were used to evaluate the value of RAR in assessing coronary artery stenosis, and influencing factors were determined. Results RDW and RAR levels were significantly higher in the severe group compared to the mild-to-moderate group, while ALB levels were significantly lower(P<0.05).RDW and RAR were positively correlated with the Gensini score, whereas ALB was negatively correlated(P<0.05).The AUC(95%confidence interval[CI])for RDW,ALB,and RAR in assessing coronary artery stenosis were 0.723(0.678-0.773),0.816(0.766-0.861),and 0.907(0.862-0.957),respectively.RAR had a higher AUC than RDW or ALB alone. The severe group exhibited higher levels of brain natriuretic peptide, neutrophil count, monocyte count,high-sensitivity C-reactive protein, and a greater proportion of patients with a history of diabetes compared to the mild-to-moderate group(P<0.05).Binary logistic regression analysis identified brain natriuretic peptide≥141.09 pg/mL(OR=2.555,95%CI:1.272-5.133),high-sensitivity C-reactive protein≥23.32 mg/L(OR=2.401,95%CI:1.379-4.182),and RAR≥0.39(OR=4.067,95%CI:1.916-8.633)as independent risk factors for coronary stenosis severity(P<0.05).Conclusion RAR is closely associated with the degree of coronary artery stenosis in perimenopausal female patients with coronary heart disease.Elevated RAR can serve as an important indicator for evaluating coronary stenosis severity.
