Observation on Placental Vascular Parameters,Ultrasound Manifestations of Uterine Artery Blood Flow and Characteristics of Placental Tissue Immune Cells in Patients with Pregnancy-Induced Hypertension of Different Pregnancy Outcomes

Abstract:Objective To explore the placental vascular parameters,ultrasound manifestations of uterine artery blood flow,and characteristics of placental tissue immune cells in patients with pregnancy-induced hypertension(PIH)with different pregnancy outcomes.Methods A retrospective case-control study was conducted.Eighty-seven pregnant women with PIH diagnosed in Chang'an Hospital from March 2021 to March 2024 were selected.Among them,51 patients had adverse pregnancy outcomes(adverse group),and 36 pregnant women with PIH had a normal delivery(control group).The placental vascular parameters,uterine artery blood flow parameters,and the expression of immune cells in the placental basal decidua of the two groups of pregnant women were compared,and a logistic regression model was established to analyze the relationship between various indicators and adverse pregnancy outcomes in pregnant women with PIH.Results The measured values of placental vascular volume,vascular index(VI),flow index(FI),and vascular flow index(VFI)in the adverse group were lower than those in the control group,and the differences were statistically significant(P<0.05).The pulsatility index(PI),resistance index(RI),and systolic/diastolic ratio(S/D)of the uterine artery in the adverse group of pregnant women were higher than those in the control group,and the differences were statistically significant(P<0.05).The positive expression rates of CD3,CD8,CD56,and CD68 in the placental tissue of mothers in the adverse group were higher than those in the control group,and the differences were statistically significant(P<0.05).The results of the logistic regression analysis model showed that an increase in placental vascular volume and VFI was beneficial to reducing the risk of adverse pregnancy outcomes in pregnant women with PIH.An increase in uterine artery RI,positive expressions of CD3,CD8,CD56,and CD68 in placental tissue,and severe preeclampsia were independent risk factors for adverse pregnancy outcomes in pregnant women with PIH(P<0.05).Conclusion A decrease in placental vascular volume and VFI,an increase in uterine artery blood flow resistance,and abnormal expression of immune cells in placental tissue in pregnant women with PIH all increase the risk of adverse pregnancy outcomes.
