Analysis of the Correlation between TRAF6 Gene Polymorphism,Helicobacter pylori Infection and Upper Gastrointestinal Bleeding Complicated with Liver Cirrhosis

Abstract:Objective To explore the correlation between tumor necrosis factor receptor-associated factor 6(TRAF6)gene polymorphisms and Helicobacter pylori(Hp)infection and liver cirrhosis complicated by upper gastrointestinal bleeding(UGIB).Methods A total of 130 patients with liver cirrhosis and UGIB who were treated in Nantong Second People's Hospital from January 2020 to January 2023 were selected as the research group(n=130)for this study.Meanwhile,130 patients with simple liver cirrhosis who received corresponding treatment during the same period were selected as the control group(n=130).The basic information of the two groups of patients was collected,and the genotype distribution of the TRAF6 gene rs5030445 locus,the Hp infection genotype distribution,and the causes of bleeding in liver cirrhosis with UGIB were compared between the two groups.Results There was no statistically significant difference in the genotype distribution at the TRAF6 gene rs5030445 locus between the two groups of patients(P>0.05),indicating population representativeness.However,the proportion of the AA genotype at the TRAF6 gene rs5030445 locus in the research group was higher than that in the control group(P<0.05).Logistic regression analysis showed that the esophageal varices grade,Child-Pugh grade,the AA genotype of the TRAF6 gene locus,and positive Hp infection were risk factors for liver cirrhosis with UGIB(P<0.05).The number of patients with Hp infection genotypes cagA and iceA in the research group was significantly higher than that in the control group(P<0.05).In patients with liver cirrhosis and UGIB who were Hp positive,the main bleeding cause was variceal rupture,followed by ulceration,and the proportions of both causes were significantly higher than those of Hp negative patients(P<0.05).Conclusion The esophageal varices grade,Child-Pugh grade,the AA genotype of the TRAF6 gene locus,and positive Hp infection are risk factors for liver cirrhosis with UGIB.Meanwhile,the main Hp infection genotypes in patients with liver cirrhosis and UGIB are cagA and iceA,and the main bleeding causes are variceal rupture,followed by ulcerative bleeding.
