Diagnostic Value of HR-MRI and Head and Neck CT Angiography for Acute Cerebral Infarction and Its Intracranial Artery Stenosis

Abstract:Objective To investigate the diagnostic value of high-resolution magnetic resonance imaging(HR-MRI)and computed tomography angiography(CTA)of the head and neck in acute cerebral infarction and intracranial arterial stenosis.Methods A retrospective study was carried out.Sixty-four patients with acute cerebral infarction who visited Hubei Jianghan Oilfield General Hospital from January 2020 to June 2023 were selected.All patients underwent HR-MRI,CTA,and digital subtraction angiography(DSA)examinations.The DSA examination results were taken as the"gold standard"to analyze the differences in HR-MRI and CTA in diagnosing carotid and intracranial artery lesions.Results The Kappa values of HR-MRI and CTA in diagnosing the degree of carotid artery stenosis in acute cerebral infarction compared with DSA were 0.799 and 0.756 respectively(P<0.05),indicating good consistency.There was no statistically significant difference in the accuracy of HR-MRI and CTA in diagnosing the degree of carotid artery stenosis in acute cerebral infarction(P>0.05).The detection rates of anterior cerebral artery,middle cerebral artery,and vertebral artery stenosis lesions by HR-MRI were 33.59%,47.66%,and 20.31%respectively,which were significantly higher than those by CTA(21.88%,27.34%,10.94%)(P<0.05).There was no statistically significant difference in the detection rate of basilar artery stenosis between HR-MRI and CTA(P>0.05).The consistency between the degree of intracranial arterial stenosis diagnosed by HR-MRI and DSA was good,with a Kappa value of 0.823(P<0.05).The consistency between the degree of intracranial arterial stenosis diagnosed by CTA and DSA was relatively poor,with a Kappa value of 0.541(P<0.05).The accuracy rate of HR-MRI in diagnosing the degree of intracranial arterial stenosis was 90.63%,which was significantly higher than that of CTA(79.24%)(P<0.05).Conclusion There is no significant difference between HR-MRI and head and neck CT angiography in the diagnosis of carotid artery stenosis in acute cerebral infarction.In the diagnosis of intracranial vascular stenosis,HR-MRI has a higher diagnostic value.
