Evaluation of Tumor Invasion and Metastasis by CT Imaging Features of Peripheral Lung Cancer and Its Correlation with Apoptosis-Related Factors

Abstract:Objective To evaluate the CT imaging features of peripheral lung cancer in assessing tumor invasion and metastasis and their relationship with apoptosis-related factors.Methods A prospective study was conducted.Ninety-eight patients with peripheral lung cancer admitted to Hubei Jianghan Oilfield General Hospital from January 2019 to December 2022 were included as research subjects.According to the degree of infiltration,the patients were classified into the precursor lesion group(26 patients),the microinvasion group(35 patients),and the invasive group(37 patients).According to the mediastinal lymph node metastasis,the patients were divided into the lymphatic metastasis group(69 patients)and the non-lymphatic metastasis group(29 patients).The CT imaging features and the differences in Bcl-2 and caspase-3 levels among patients with different degrees of infiltration and lymphatic metastasis were compared.Results Statistically significant differences were found in lesion boundary,spiculation sign,vacuole sign,three-dimensional shape,lobulation sign,air bronchogram sign,and pleural indentation among the precursor lesion group,microinvasion group,and invasive group(P<0.05).Statistically significant differences were found in lesion boundary,spiculation sign,vacuole sign,three-dimensional shape,lobulation sign,air bronchogram sign,and pleural indentation between the lymphatic metastasis group and the non-lymphatic metastasis group(P<0.05).Statistically significant differences were found in Bcl-2 and caspase-3 levels among the precursor lesion group,microinvasion group,and invasive group(P<0.05).Statistically significant differences were found in Bcl-2 and caspase-3 levels between the lymphatic metastasis group and the non-lymphatic metastasis group(P<0.05).Correlation analysis showed that Bcl-2 was positively correlated with lesion boundary(blurred),spiculation sign,vacuole sign,three-dimensional shape(spherical),lobulation sign,air bronchogram sign,and pleural indentation,while caspase-3 was negatively correlated with lesion boundary(blurred),spiculation sign,vacuole sign,three-dimensional shape(spherical),lobulation sign,air bronchogram sign,and pleural indentation.Conclusion The CT imaging features of peripheral lung cancer are of significance in assessing tumor invasion and metastasis and show a significant correlation with apoptosis-related factors Bcl-2 and caspase-3.
