Comparison of the Results of Gastroscopic Biopsy and Endoscopic Resection Pathological Examination for Gastric Mucosal Intraepithelial Neoplasia and Analysis of Influencing Factors for the Differences

Abstract:Objective To compare the results of gastroscopic biopsy and endoscopic resection pathological examination for gastric mucosal intraepithelial neoplasia(GIN)and explore the influencing factors for the differences.Methods A retrospective analysis was conducted on 80 patients with GIN admitted to Dezhou Hospital,Qilu Hospital,Shandong University from December 2019 to December 2022.The patients underwent gastroscopic biopsy and endoscopic resection pathological examination,and the examination results were compared.The lesion size,location,morphology,and the conditions of mucosal surface congestion,surface nodular changes,and mucosal ulcers were recorded.The relevant factors contributing to the differences between the results of gastroscopic biopsy and endoscopic resection pathological examination were analyzed.Results The diagnostic accuracy rate of gastroscopic biopsy for low-grade GIN was 65.21%(15/23),68.89%(31/45)for high-grade GIN,100.0%(12/12)for early gastric cancer(EGC),and the overall diagnostic accuracy rate was 72.50%(58/80).GIN diameter>2.0 cm,the lesion located in the proximal stomach,redness on the lesion surface,and surface depression/ulcer of the lesion were the influencing factors for the differences between the two examination methods(P<0.05).The results of multivariate regression analysis showed that lesion size>2 cm(OR=2.147,95%CI:1.591-2.898),the lesion site in the proximal stomach(OR=3.206,95%CI:1.915-5.368),redness of the lesion(OR=2.552,95%CI:1.675-3.890),and surface depression/ulcer of the lesion(OR=4.655,95%CI:2.700-8.028)were the influencing factors for the differences in GIN examination results(P<0.05).Conclusion Gastroscopic biopsy for GIN may lead to missed diagnosis of high-grade GIN.For lesions with a diameter>2 cm,lesions located in the proximal stomach,with redness,and surface depression or ulceration,endoscopic resection and pathological examination are recommended.
