The Evaluation Value of Erythrocyte Sedimentation Rate,CA 125,High-Sensitivity C-Reactive Protein,and D-Dimer for Prognosis in Patients with Tuberculous Tuberculous Multiple Serous Cavity Effusion

Abstract:Objective To investigate the value of erythrocyte sedimentation rate(ESR),carbohydrate antigen 125(CA125),high-sensitivity C-reactive protein(hs-CRP),and D-dimer(D-D)in evaluating prognosis of patients with tuberculous multiple serous cavity effusion(TMSCE).Methods The study included 102 patients diagnosed with TMSCE from January 2021 to January 2024.The patients were divided into two groups based on their prognosis,with 60 patients having good prognosis and 42 having poor prognosis.The general information and laboratory data(ESR,CA125,hs-CRP,D-D,WBC,and PLT)of the two groups were compared.Multivariate regression analysis was used to identify the prognostic factors of TMSCE patients.Receiver operating characteristic(ROC)curves were drawn to analyze the evaluation value of ESR,CA125,hs-CRP,D-D,and the combination of these four indicators for the prognosis of TMSCE patients.Results Comparing the differences in age,gender,etc.among patients with different prognoses,the P-values were all>0.05,indicating no statistically significant differences and comparability.The laboratory indicators ESR,CA125,hs-CRP,and D-D showed that the levels in the poor prognosis group were higher,with statistically significant differences(P<0.05).The multivariate Logistic regression analysis showed that ESR,CA125,hs-CRP,and D-D were independent influencing factors of poor prognosis in TMSCE patients.The ROC analysis showed that the area under the curve(AUC)of ESR,CA125,hs-CRP,D-D,and the combined detection in evaluating the prognosis of TMSCE patients was 0.952,with a sensitivity of 91.57%and a specificity of 81.95%,which was higher than that of any single indicator.Conclusion The levels of ESR,CA125,hs-CRP,and D-D are closely related to the poor prognosis of TMSCE patients.Monitoring these indicators dynamically can help identify the risk of poor prognosis early on and adjust treatment plans in time to reduce the risk of complications and improve the clinical outcomes of patients.
