The Significance of Peripheral Blood ox-LDL and D-D Levels in Assessing the Condition and Prognosis of Patients with Cerebral Infarction

Abstract:Objective Exploring the value of peripheral blood oxidized low-density lipoprotein(ox-LDL)and plasma D-dimer(D-D)expression levels in evaluating the condition and prognosis of patients with acute cerebral infarction(ACI).Methods Retrospective analysis of clinical data of 112 ACI patients at Ningde City Hospital affiliated with Ningde Normal University from June 2022 to June 2023.The National Institutes of Health Stroke Scale(NIHSS)score was used to evaluate the condition,with 0-4 points being classified as mild group(n=25),5-15 points being classified as moderate group(n=63),and 16-42 points being classified as severe group(n=24);Patients were divided into three groups based on their age:young group(n=19),middle-aged group(n=59),and elderly group(n=34);According to the modified Rankin Scale(mRS)score after 3 months,there were 62 cases in the good prognosis group(mRS score 0-2)and 50 cases in the poor prognosis group(mRS score 3-6).Collect general information of patients before treatment,as well as peripheral blood ox-LDL,D-D expression levels,Glasgow Coma Scale(GCS),et al.Compare the expression levels of ox-LDL and D-D in different groups,use logistic regression analysis to identify prognostic factors in ACI patients,and draw receiver operating characteristic(ROC)curves to evaluate the predictive value of ox-LDL and D-D expression levels for poor prognosis in ACI patients.Results The levels of ox-LDL and D-D in mild ACI patients were lower than those in moderate and severe ACI patients(P<0.05),and the levels of ox-LDL and D-D in moderate ACI patients were lower than those in severe ACI patients(P<0.05).The levels of ox-LDL and D-D in young ACI patients were lower than those in the middle-aged and elderly groups(P<0.05),and the levels of ox-LDL and D-D in middle-aged ACI patients were lower than those in the elderly group(P<0.05).The levels of ox-LDL,D-D,and NIHSS score in the good prognosis group were lower than those in the poor prognosis group(P<0.05),and the GCS score in the good prognosis group was higher than that in the poor prognosis group(P<0.05).The results of multiple logistic regression analysis showed that NIHSS score,GCS score,ox-LDL,and D-D were the influencing factors on the prognosis of ACI patients(P<0.05).The ROC curve results showed that the area under the ox-LDL curve(AUC)was 0.708(95%CI:0.608-0.809),the D-D AUC was 0.758(95%CI:0.665-0.851),and the combined detection AUC of the two was 0.813(95%CI:0.729-0.896).Conclusion The more severe the condition of ACI patients,the higher the levels of ox-LDL and D-D.NIHSS score,GCS score,and ox-LDL and D-D levels are independent predictive factors for poor prognosis in ACI patients,and ox-LDL and D-D levels have high predictive power for patient prognosis.
