The Effect of Dynamic Cross-Pinning System and Inverted Triangle Fixation on Unstable Femoral Neck Fractures in Young and Middle-Aged Patients Based on Propensity Score Matching

Abstract：Objective To compare the efficacy of dynamic cross-pinning system and inverted triangle fixation for the treatment of unstable femoral neck fractures in young and middle-aged people based on propensity score matching.Methods A total of 181 young and middle-aged patients with unstable femoral neck fracture were divided into group observation(n=96)and group control(n=85)according different surgical methods.Propensity score matching(PSM)was further adopted,and 54 patients were successfully matched in each group.The observation group was treated with dynamic cross-nailing system of bone neck,and the control group was treated with inverted triangle fixation.The relevant indexes of operation,fracture healing time,complication rate,surgical reduction outcome,and hip function score were evaluated in the two groups.Results The surgical-related indexes and fracture healing time in the observation group were lower than those in the control group(P＜0.05).The incidence of surgical complications in the observation group was lower than that in the control group(P＜0.05).The surgical excellent rate in the obser-vation group was higher than that in the control group(P＜0.05).Three months postoperatively,the Harris hip function score in the observation group was higher than that in the control group(P＜0.05).Conclusion Compared with the inversion triangle fixa-tion,dynamic cross-pinning system is more effective in the treatment of middle-aged and young adults with unstable femoral neck fractures,which can reduce the surgical trauma of patients,reduce surgical complications,promote fracture healing,improve the level of bone metabolism of patients,and enhance hip mobility.
