Predictive Value of Serum P Ⅲ NP and CHE Levels in the Prognosis of Patients with Acute Decompensated Heart Failure

Abstract：Objective To investigate the predictive value of serum procollagen Ⅲ N-terminal peptide(P Ⅲ NP)and cholinesterase(CHE)levels in the prognosis of patients with acute decompensated heart failure(ADHF).Methods A total of 132 ADHF patients treated at the Department of Critical Care Medicine,PLA Air Force Medical University 986 Hospital from January 2021 to May 2023 were enrolled and classified into a poor prognosis group(46 patients)and a good prognosis group(86 patients)according to the 6-month prognostic outcome.Enzyme-linked immunosorbent assay was used to detect serum P Ⅲ NP and CHE levels.Factors of poor prognosis in ADHF patients and the predictive value of serum P Ⅲ NP and CHE levels for poor prognosis were analyzed.Results At 6-month follow-up,the incidence of poor prognosis was 34.85％(46/132)in 132 ADHF patients.Serum P Ⅲ NP levels were increased,and CHE levels were decreased in the poor prognosis group compared with the good prognosis group(P＜0.05).Independent risk factors for poor prognosis in patients with ADHF were increasing age,atrial fibrillation,New York Heart Association cardiac function class≥Ⅲ,elevated N-terminal pro-B-type natriuretic peptide,and elevated P Ⅲ NP,and independent protective factors were elevated left ventricular ejection fraction and CHE(P＜0.05).The area under the curve of serum P Ⅲ NP combined with CHE levels for predicting poor prognosis in ADHF patients was greater than that of serum P Ⅲ NP and CHE levels alone(P＜0.05).Conclusion Elevated serum P Ⅲ NP levels and de-creased CHE levels in ADHF patients are independent influencing factors of prognosis,and serum P Ⅲ NP combined with CHE levels have a high predictive value for the prognosis of ADHF patients.
