The Relationship Between Serum sTREM-1,8-OHdG,HBP and the Severity of Pneumonia and their Prognostic Value in Patients with Severe Pneumonia

Abstract:Objective To investigate the relationship between serum soluble triggering receptor expressed on myeloid cells-1 (sTREM-1),8-hydroxy-deoxyguanosine (8-OHdG),heparin-binding protein (HBP) and the severity of pneumonia and their prognostic value in patients with severe pneumonia (SP).Methods From May2020 to January2023,201 elderly SP patients diagnosed and treated in department of geriatrics,Xingtai metallurgical hospital were collected as the observation group.According to the prognosis,they were grouped into good prognosis group and poor prognosis group;meantime,186 eld-erly mild pneumonia were gathered as the control group.Enzyme linked immunosorbent assay (ELISA) was applied to detect serum levels of sTREM-1,8-OHdG,and HBP;Pearson correlation was applied to analyze the correlation between serum sTREM-1,8-OHdG,HBP and baseline data in elderly SP patients with poor prognosis;Logistic regression analysis was used to analyze factors affecting patient prognosis;Predictive value of serum sTREM-1,8-OHdG,and HBP for patient prognosis by ROC curve analysis.Results The serum levels of sTREM-1,8-OHdG,and HBP in the observation group were higher than those in the control group (P<0.05).The sTREM-1,8-OHdG,HBP,APACHE Ⅱ score,white blood cell count,PaCO2,PCT of the poor prognosis group were higher than those of the good prognosis group,while PaO2 and oxygenation index were lower than those of the good prognosis group (P<0.05).Serum sTREM-1,8-OHdG,and HBP in patients with poor progno-sis were positively correlated with APACHE Ⅱ score,white blood cell count,PaCO2,PCT,and negatively correlated with PaO2 and oxygenation index (P<0.05).STREM-1,8-OHdG and HBP were independent risk factors for poor prognosis of elderly SP patients (P<0.05).The area under the curve of serum sTREM-1,8-OHdG and HBP alone and jointly predicting poor prognosis of elderly SP patients was 0.861,0.744,0.864 and 0.952,respectively.Conclusion Serum sTREM-1,8-OHdG,HBP are closely related to the progression of pneumonia,and combined detection has important predictive value for poor prognosis of SP.
