The Correlation among Preconception Body Mass Index,Gestational Weight Gain and Vaginal Trial Delivery Transferring to Cesarean Section,Macrosomia in Primipara

Abstract:Objective To investigate the preconception body mass index (BMI) and gestational weight gain (GWG) of primipara,and explore their correlation with vaginal trial delivery transferring to cesarean section and macrosomia.Methods A total of 248 parturients who underwent vaginal trial labor in our hospital were divided into 21 cesarean section group (va-ginal trial delivery transferring to cesarean section) and 227 vaginal delivery group by delivery method.According to the pres-ence of macrosomia,the patients were divided into 18 macrosomia group and 230 non-macrosomia group;According to precon-ception BMI,the patients were divided into 43 low body weight group,181 normal body weight group and 24 overweight/obese group;According to GWG,there were 42 cases of insufficient growth group,107 cases of normal growth group and 99 cases of excessive growth group.Multivariate Logistic regression was used to analyze the effects of preconception BMI and GWG on va-ginal trial delivery transferring to cesarean section and macrosomia in primipara.Results The rates of overweight/obesity and excessive increase of GWG in cesarean section group were higher than those in vaginal delivery group (P<0.05) .The pro-portion of overweight/obesity and excessive increase of GWG in macrosomia group was higher than that in non-macrosomia group (P<0.05).Logistic regression analysis showed that preconception overweight/obesity and excessive increase of GWG were risk factors for vaginal trial delivery transferring to cesarean section in primipara (P<0.05).Logistic regression analysis showed that preconception overweight/obesity and excessive increase of GWG were risk factors for macrosomia in primipara (P<0.05).Conclusion Preconception BMI and GWG are closely related to the risk of vaginal trial delivery transferring to ce-sarean section and macrosomia.Preconception overweight/obesity and excessive increase of GWG can significantly increase the risk of vaginal trial delivery transferring to cesarean section and macrosomia in primipara.It is recommended to strengthen pre-pregnancy and pregnancy weight management for women of childbearing age,and reasonably control preconception BMI and pregnancy weight gain to improve maternal and infant outcomes.
