Changes in Hormone Receptor Expression in Primary and Recurrent Metastatic Breast Cancer and Assessment of Patient Survival

Abstract:Objective To investigate the expression of AR,ER and PR in primary and metastatic breast cancer and its predictive value for survival of breast cancer patients.Methods Primary and metastatic tissues of100 patients with breast cancer recurrence and metastasis admitted to the The Fifth Department of General Surgery at Handan Central Hospital from January 2018 to January 2020 were collected.AR,ER and PR were detected by immunohistochemical staining,and HE stai-ning was observed.The expressions of AR,ER and PR in the primary and metastatic breast cancer were compared with those in the survival and death groups,and the predictive value of AR,ER and PR alone,the correlation between AR expression and ER and PR was analyzed and combined detection on the survival of breast cancer patients was evaluated by ROC curve.Results There was no difference between ER positive expression rate (55.00%) and PR positive expression rate (55.00%) in primary breast cancer and ER positive expression rate (51.67%) and PR positive expression rate (46.67%) in metastatic breast cancer (P>0.05).The positive expression rate of AR in primary breast cancer (33.33%) and recurrent metastatic breast cancer (51.67%) was significantly different (P<0.05).The expression consistency rates of AR,ER and PR were 58.33%,63.33% and 60.00%,respectively,in the primary and recurrent breast cancer groups.The OS for AR+/AR+,AR+/AR-,AR-/AR+and AR-/AR-were 13,31,16 and 42 months,respectively (P<0.05).The OS of ER+/ER+,ER+/ER-,ER-/ER+and ER-/ER-were 42,37,23 and 15 months (P<0.05).The OS of PR+/PR+,PR+/PR-,PR-/PR+and PR-/PR-were 42,36,23 and 22 months (P<0.05).The positive expression rates of AR,ER and PR in the death group were lower than those in the survival group (P<0.05).The expression of AR was posi-tively correlated with ER and PR (r=0.275,0.314,P<0.05).The AUC of AR,ER and PR combined detection was 0.873,which was higher than that of AR,ER and PR alone detection (P<0.05).Conclusion The expression consistency of AR,ER and PR in the primary and recurrent metastatic breast cancer was high,and the expression of AR was positively correlated with ER and PR,but patients with negative AR were more likely to have breast cancer recurrence and metastasis,and the combined diagnosis of AR,ER and PR had higher predictive value for the survival of breast cancer patients.
