Analysis of the Efficacy and Recurrence Risk Factors of Local Microwave,Fluconazole Combined with Miconazole Nitrate in the Treatment of Fungal Otitis Externa

Abstract：Objective To investigate the efficacy and recurrence risk factors of local microwave,fluconazole com-bined with miconazole nitrate in the treatment of fungal otitis externa (FOE).Methods 100 patients with FOE admitted to Sanshui District People's Hospital in Foshan City from January 2023 to January 2024 were The patients were divided into an ob-servation group (n=50) and a control group (n=50) according to the random number table method.Both groups received microwave therapy.The control group received fluconazole injection ear bath after therapy,while the observation group re-ceived topical application of miconazole nitrate cream.Compare the clinical efficacy and inflammatory factors between two groups,record the recurrence status of patients,and use logistic regression analysis to analyze the risk factors for recurrence after FOE treatment.Results The total effective rate of the observation group was higher than that of the control group,and the recurrence rate was lower than that of the control group 6 months after treatment (P<0.05);After treatment,the levels of interleukin-6 (IL-6),tumor necrosis factor-α(TNF-α),and C-reactive protein (CRP) in both groups were lower than those in the control group,and the levels in the observation group were lower than those in the control group (P<0.05);Multivari-ate analysis showed that age≥60 (OR=5.800),combined with diabetes (OR=6.813),combined with chronic nose and throat diseases (OR=6.333),and the habit of wearing headphones for a long time (OR=4.760),the habit of digging ears (OR=8.478),the storage time of cotton swabs after opening up (OR=16.200) and one month (OR=27.000),the kitch-en where cotton swabs were stored (OR=35.250),and the bathroom (OR=7.344) were the high-risk factors for the recur-rence of fungal otitis externa (P<0.05).Conclusion After treatment with local microwave,fluconazole and miconazole ni-trate,FOE can improve the efficacy,improve inflammatory reaction and reduce disease recurrence.The recurrence of FOE is closely related to age,diabetes,chronic nose and throat diseases,the habit of wearing earphones for a long time,the habit of ear digging,the storage time of cotton swabs after opening,the storage environment of cotton swabs,and the strict compliance with the doctor's advice on the use of ear drops.
