The Value of Thromboelastography and Coagulation Function in Pregnant Women with Placenta Previa Hemorrhage

Abstract:Objective To assess the value of thromboelastography (TEG) in patients with placenta previa hemor-rhage and to compare it with traditional coagulation function testing to improve the detection rate of hypocoagulable states,optimize the use of blood products,and improve intraoperative and postoperative indicators. Method 200 patients with placenta previa with hemorrhage admitted to Zigong Maternal and Child Health Hospital from September 2019 to January 2024 were selected and randomly divided into an experimental group and a control group,100 cases each. The experimental group used TEG to guide blood transfusion strategies,while the control group used traditional coagulation function testing. The differences in terms of detection rate of hypocoagulable state,intraoperative blood product consump-tion,intraoperative and postoperative monitoring indicators,and rebleeding rate were compared between the two groups. Result TEG testing showed that the hypocoagulable state detection rate in the experimental group was considerably higher compared to the control group (56.5% vs 25.5%,P<0.05),and in the experimental group,the required plasma,platelets,red blood cells,and cryoprecipitate transfusion volumes were significantly lower than those in the control group (P<0.05). In terms of intraoperative and postoperative monitoring indicators,the intraoperative blood loss,postopera-tive hemoglobin level,and hospitalization time in the experimental group were significantly lower than the control group. The rate of rebleeding in the experimental group was notably reduced compared to that in the control group (6% vs 19%,P=0.027). Conclusion The results of this study showed that,for patients with placenta previa hemorrhage,using TEG technology to guide blood transfusion strategies could effectively improve the detection rate of hypocoagulable states,re-duce the use of blood products,improve intraoperative bleeding control,postoperative hemoglobin levels,and length of hospital stay,and significantly reduce the risk of rebleeding.
