Imaging Study of Screw Canal Simulation of Lumbar Pedicle Screw Place-ment Based on CT 3D Visualization

Abstract:Objective To obtain the imaging parameters of screw placement in traumatic lumbar vertebrae before pedicle screw placement by CT three-dimensional(3D)visualization reconstruction and interactive computer simulation tech-nology.Methods Thin-slice CT scan data of 125 patients with lumbar spine fractures from June 2020 to December 2022 were selected,and 3D model reconstruction was carried out using computer software.Digital simulation technology was used to sim-ulate pedicle screw trajectory,measure pedicle parameters,and observe screw positions after surgery.Results There was no significant difference between the left and right side of the pedicle height(PH),pedicle width(PW),pedicle axis length,pedicle cancellous bone height(CCH),pedicle cancellous bone width(CCW),pedicle transverse angle(PTA)and pedicle sagittal angle(PSA)in 125 patients with lumbar spine fractures(P>0.05).There were significant differences in PTA and PSA on the same side among different vertebral bodies(P<0.05).In the 125 patients with lumbar vertebral fractures,ima-ging results of screw trajectory simulation of lumbar pedicle screw placement based on CT 3D visualization showed that there were good screw placement positions and no serious complications.Conclusion CT 3D visualization reconstruction and inter-active computer simulation technology can accurately simulate and calculate preoperative image parameters(the optimal screw entry point,screw placement angle,screw trajectory).The use of preoperative imaging model data to guide the clinical intrao-perative screw placement pathways and modes can effectively prevent the occurrence of surgical complications.
